Public Notice of Change of Surname for Official Records

 



This is to formally notify the general public, educational institutions, banks, embassies, and all relevant authorities that the children of Mrs. Pachucs Okolo Oluchukwu Olive shall henceforth bear the surname Pachucs, in place of Okolo, to reflect their father’s family name.

All previous documents and records bearing their former surname remain valid and legally refer to the same individuals. This notice is issued for official documentation, identification, and record purposes, and the general public is hereby advised to take note.
